Community Outreach Specialist
South Side Thrive Collaborative is a collective impact initiative with a mission to support residents of the South Side of Columbus in living healthy, fulfilling lives by strengthening the collective efforts of community partners. As the backbone organization for SSTC, Nationwide Children’s will employ this position but the performance expectations will be set by SSTC leadership. The Community Outreach Specialist contributes to the development, improvement, and implementation of the Neighborhood Navigator Program, a service model that assists by providing information on appropriate resources and referrals. The Community Outreach Specialist provides referrals for essential support services surrounding eviction prevention, food assistance, employment training, transportation, and other needs. Must be able to work occasional non-traditional hours, including some evenings and Saturdays.
Responsibilities
• Coordinates and conducts community outreach to reach target population, including attending events and maintaining a regular schedule of in-person engagement at partner locations.
• Develops relationships with community providers to facilitate more efficient referrals, collaborates to increase awareness of critical resources, identifies barriers, and advocates for improving service delivery and other strategies.
• Provides information on appropriate resources and coordinates referrals. Identifies and develops opportunities to engage community-based non-profits and provide information on available community resources.
• Maintains awareness of services, resources, and programs within the community, including associated eligibility requirements and application processes. Guides individuals through screening and documentation process to determine eligibility and assists in completing program applications.
• Collects data and provides documentation on individuals served on a consistent basis. Produces regular reports.
• Coordinates trainings for partners to increase awareness of critical resources.
