About the Organization: South Side Thrive Collaborative is a collective impact initiative with a mission to support residents of the South Side of Columbus in living healthy, fulfilling lives by strengthening the collective efforts of community partners. As the backbone organization for SSTC, Nationwide Children’s will employ this position, but the performance expectations will be set by SSTC leadership.
General Position Description: The Neighborhood Navigator will contribute to the development of, and be responsible for the implementation of, the Neighborhood Navigator Pilot Program. This program involves conducting community outreach and supporting South Side residents by making referrals to essential support services including housing, food assistance, employment services, and other needs as identified
Schedule: (Temporary, Part Time) Position will Conclude June 30, 2022
Monday-Friday: 8am – 5pm (Available one Evening: Monday-Thursday and Saturday’s)
Responsibilities
- Coordinate and conduct outreach at social service provider locations (ex: food pantries, churches, community centers). Organize outreach events and collaborate with community partners to engage community members.
- Identify and engage with individuals who may benefit from a referral to community resources.
- Provide support and linkages to community resources by creating an individualized action plan with individuals served.
- Guide individuals through a screening and documentation process to determine eligibility for assistance programs. Assist individuals in completing program applications as necessary.
- Engage subset of individuals in on-going coaching by partnering to set goals, develop strategies and reduce barriers to meet goals.
- Maintain awareness of services, resources, and programs within the community.
- Maintain familiarity with resources eligibility requirements and application processes.
- Develop relationships with community providers to facilitate more efficient referrals.
- Collect data and documentation on individuals served. Produce regular reports.
